Keys in Quests Issue

Hi - very often when looting quest items, there is a delay between looting said item and it appearing in my character's inventory.  This has been problematic with keys; twice now (Gol'dir quest and Stones of Binding) I have looted the relevant quest key without it appearing in my inventory or key ring.  The quest registers the key as "acquired" though I do not have it and cannot use it.  I tried abandoning both quests, but when I re-acquired the quests, the key remained "collected" for quest purposes, though I had no key.  I am unable to progress in either quest at this point.
