I think this is paranoid stuff, however if it's currently how their detection system works then this should definately be highlighted. Could a GM please confirm? Should we be taking screenshots of our items going up? Should we make tickets to GM first confirming we are putting an item up?

You are reading far too much into it. Normal PvP is fine. Safespoting/organised honor farm/exploiting - That will get you banned.

Meetings stones are just that. Stones that you meet other people at. It is not broken or bugged, this is how they were intended. It was only changed in later expansions to give the summon ability. Edit: (TBC) Patch 2.0.1 (05-Dec-2006): Meeting Stones now function similar to a Warlock Summon spell.

On Nostalrius, MCing people off boats and through BG portals was considered an offence. You would normally first get a warning on your account. Also people BEING MCed through BG portals could trigger a false positive, this could be devastating to a player trying to get ranks in PvP. That being said, if you did get MCed out of a BG you probably deserved it for standing AFK so close to the portal.
