(propably stupid) questions about innervate

- Since it increases the mana regen by 400%, this means we get 400% of the mp5 regen we have from items?

With spirit,
- does it give 400% manaregen from the spirit regen inside 5sec rule (the 15% from talents + another 15% from gear) and after 5 sec 400% from all the spirit regen?

And finally, how it calculates the "while casting" regen? Assuming I've understood correct mp5 regen is not affected by the 5sec rule. So assuming I'm casting I will get 400% from the mp5 regen and another 400% from the 15% regen? Or am I completly lost with this?

It means that you have no penalty to spirit-based mana regeneration while casting (no 5-second rule), and that you get a 400% bonus to spirit-based mana regeneration. It does not affect mp5 regen from items.

Here's a quick guide to regen:

Mp5 gives you X mana every 5 sec. Mp5 does not benefit or scale with other stats.
Mp5 is a 'straight forward' stat. No tricks or calculations needed.

Mana regen from spirit works quite differently. This form of regen is only active after five sec of not casting spells. This is known as the "5 sec rule".
This regen will tick every 2 sec untill you succesfully cast a spell and reset the 5sec rule. 
Mana regen from spirit also scales coefficiently with intellect. For example at 200 intellect you only get the benefit from 66% of your spirit. (see table below)

So, below 450 intellect your spirit will work at a reduced capacity. Just above 450 it's at a 1:1 ratio and above 450 intellect your spirit will work at an increased capasity. You may have heard talk that spirit becomes a very special stat for some classes end-game? This is why. Because the better gear, buffs, consumes etc you have, the more you will benefit from spirit.

Furthermore, as a druid and priest you get keep 15% of your mana regen from spirit while casting through a talent - and further 15% from the tier2 3set bonus.
This means that that % of your mana regen from spirit will still be active while casting and you do not have to wait for the 5 sec rule to benefit from spirit. However, if you do manage to still activate the 5 sec rule you'll get the full benefit from spirit and not just the % from talents and set-bonus.
Please note that with 15% from talents, you will only benefit 85% after activationg the 5 sec rule, as you would get the initial 15% anyway. Of couse you'll get the full 100% while the 5 sec rule is active, but the actuall benefit will be 85%.
If you choose to use the darkmoon card and you allready have the 15% from talents the procc from the trinket will act in a simmilar manner.

I recently did some theorycrafting on AQ40 gear with and without the 3 set bonus because I was curious about what items to prioritize to get first.

As an alliance (this is imporntant to specify because of Blessing of Kings) I did the calculations for 3 scenarios:

1. Raid buffed. All availible class buff included:
- 15% from the tier2 3set bonus gives you mana regen equal to ~27 Mp5

2. Raid buffed and with consumables. All availible class buffs and consumables that benefit int and spirit included:
- 15% from the tier2 3set bonus gives you mana regen equal to ~34 Mp5

3. Raid buffed, with consumables and world buffs. All availible class buffs, consumables that benefit int and spirit and all the world buffs:
- 15% from the tier2 3set bonus gives you mana regen equal to ~45 Mp5

I think druid spirit regen is something like:

Spirit / 5 + 15

Per tick. You get a tick every 2 seconds.

Within the 5 second rule you use a percentage of that: 15% with just the talent and 30% with the t2 3 piece.

So with 500 spirit and 30% regen you get 34.5 mana per tick or averaged to 86.25 mp5 while casting. At 100% you get 287.5 mp5 and based on the description of innervate it increases to 500% for 1,437.5 mp5 or approximately 5750 mana over the 20 seconds/10 ticks.

In comparison, a mage would regenerate 8832 mana over the course of an uninterrupted evocation (8 min cooldown vs 6 min for innervate) for 4 ticks.

Mages and priests use:

Spirit / 4 +13
